Where is my Arizona medical card? Downloading it, and what to do if you lose it.

If you are watching the mailbox, you can stop. Arizona has not issued a printed medical marijuana card since Senate Bill 1494 moved the program to electronic cards in 2019. Your card lives in the AZDHS Individual Licensing Portal, and you download it yourself. Here is where to find it, how to print it properly, and what “losing” it actually means.

When the card appears

AZDHS has up to 10 business days to issue a card once your application is complete. The physician’s certification is only half of it: you file the state application and pay the fee yourself, as described in how to get an Arizona medical card.

Until the card is approved, the portal does not show a Patient tile at all; AZDHS notes it only appears once the application has been approved. To see where things stand in the meantime:

  1. Log in to the Individual Licensing Portal.
  2. In the Programs section, select Add a new license, then Marijuana.
  3. Under Applications, choose Check Application Status.

If the status says Action Required, AZDHS needs something from you, and that is the only status in which a submitted application can be edited. The bell icon at the top of the portal shows notifications and requests.

Downloading the card, step by step

These steps follow AZDHS’s own quick-reference guide for patients and caregivers:

  1. Log in at individual-licensing.azdhs.gov and select the Individual Portal tile.
  2. Select the Marijuana program tile.
  3. Select Patient (or Caregiver, if that is your card).
  4. Open My ID Cards. It is the default landing page.
  5. Click Download PDF, then open the PDF file for a printable version.

The same page shows the card’s status and your remaining allotment. A Card History tab shows up to three years of past cards, and the left-hand menu also has your dispensary purchase history.

Printing tip from AZDHS: do not select “fit to page” when you print, and use a laser printer if you can. Keep the PDF on your phone as well as the printout. If you are not sure what a dispensary will ask for at the door, see Arizona dispensary rules for patients.

Locked out of the portal

The portal locks an account after five incorrect password attempts. At that point you cannot reset it yourself: AZDHS has to unlock it, emails you when it does, and asks you to allow one business day. If you can still log in and just want to change your password or email, the profile icon at the top right lets you do that.

“I lost my card”: usually you did not

If what you lost is a printout or the phone the PDF was saved on, the card itself is still in the portal. Log in from another device, download it again, print it again. Nothing needs replacing.

It is different if someone else may be using it. If your phone was stolen with the card on it, or you suspect fraud on your account, AZDHS has a Lost/Stolen Card Application. It issues a new card and ends the old card’s ability to buy.

  1. In the Marijuana program, open the Applications tab.
  2. Choose the Lost/Stolen Card Application tile and read the notice.
  3. Select the card ID to be replaced and upload the documents it asks for.
  4. Review, then pay to submit.

The fee is set by statute. Under ARS §36-2804.06, a cardholder who loses a card must notify AZDHS promptly, and within five days of that notice and payment of a $10 fee the department issues a new card with a new random identification number. If you have a registered caregiver, the caregiver gets a new card too.

Other changes: name, address, caregiver

Those go through a different form, the Card Change Application, also under the Applications tab. It opens with the information you originally submitted; you replace what has changed and upload supporting documents. AZDHS notes that payment is required for some changes, and that a legal name can only be changed this way. Adding or replacing a caregiver needs the patient’s request form ready to upload; the caregiver card guide explains that side.

Two more things people ask

Will the card disappear when I renew?

No. A renewed card is issued into the same portal, and the old one moves into Card History. The timing is covered in how to renew an Arizona card.

Who else can see it?

The registry is confidential under ARS §36-2810, but it is checkable by people you give the number to. See who can see that you have a card.

Once the card is on your phone, the practical questions start: first purchases, limits, storage. We cover them in your first 30 days as an Arizona patient.

For anything the portal does not answer, the AZDHS Medical Marijuana program lists its telephone number as 602-364-0857.
